Foster Care Case Management

Program Overview
Entering, re-entering or currently being in out-of-home care is a challenge for children, their biological parents, their relatives, and their foster parents.  Foster Care Case Management helps all of these client groups through the transition of out-of-home placement, visitation, and, once the situation has been resolved and the safety of the child has been assured, the eventual return of the child to his or her biological family. In some cases, when this is not possible, we help children find a new "forever family."

Services Provided
• Assessment of placement situations
• Home visitation counseling and support
• Help with obtaining needed resources
• Reunification and permanency planning meetings
• Interventions to ensure that children who enter foster care have safe, stable, and permanent
       long-term caregivers (including biological parents whenever possible)

Criteria for Referral
Clients are referred to FRC through the Missouri Division of Family Services (DFS). All Foster Care Case Management is done in conjunction with the Child Permanency Partnership (CCP).

Foster Care clients are:
• Children between the ages of newborn and 21 years old, who have been abused/neglected; who
       are homeless; whose parents have died; and/or who have no other supportive family members
• Biological parents who have abused and/or neglected their child(ren); who are homeless; who
       have no means of income; who are drug/alcohol abusers; and/or who are mentally limited
• Kinship parents who have a previous relationship wtih the child(ren) and offer a safe place to live        and grow
• Relative parents who are relatives of the child(ren) and offer a safe place to live and grow
